What is a community health needs assessment?

Every three years, Parkview Health sponsors a community health needs assessment (CHNA) to identify health needs within northeast Indiana, particularly the seven counties in which Parkview-affiliated hospitals exist. As a not-for-profit, community-owned health system, the assessment provides a valuable tool in Parkview’s strategic planning process and in the development of our community benefit plan. Each time we monitor our community’s health needs, we identify opportunities and prioritize our efforts to impact the health of our community friends and neighbors.

This data is shared with community organizations and leaders to assist in their planning and to encourage opportunities for collaboration.

The most recent assessment was completed in conjunction with the Health Services and Informatics (HSIR) group, a part of the Parkview Mirro Center for Research and Innovation. Current community health priorities for Parkview-affiliated hospitals can be found in the respective Community Health Needs Assessment reports.

What is an implementation strategy?

An implementation strategy serves as an in-depth plan for each hospital to address health needs identified through the community health needs assessment and other resources. It includes goals, objectives and action plans designed to improve the health of the local community.

The implementation strategy serves as the community health improvement plan for each hospital to address health needs identified through the community health needs assessment and other resources.
