How to apply for the Pharmacy Residency Program

Residency candidates must have a Doctor of Pharmacy degree and be eligible for pharmacist licensure in Indiana. Candidates must also register for the Matching Program and have a National Matching Services (NMS) applicant number. Candidates applying for the Parkview PGY2 Ambulatory Care Residency Program must have successfully completed an ASHP accredited PGY1 Residency.

According to guidelines by the Pharmacy Online Residency Centralized Application Service (PhORCAS), candidates must submit the following information no later than January 2, 2023:

  1. Curriculum Vitae
  2. Letter of Intent
  3. Three letters of recommendation via PhORCAS Standard Electronic Reference Form
  4. Transcript from College of Pharmacy
