Home health care is personalized healthcare provided in the comfort of your home by trained professionals who follow treatment orders from your physician. Being in a familiar setting, surrounded by family and friends, can make you or your loved one feel better during a time of illness or injury.

Who benefits from home health?

You or a loved one may benefit from receiving home health services in the following situations.

Following a hospital stay, whether you are recovering from surgery or illness. If your plan for continued recovery includes care at home, your hospital stay may even be shortened.
Changes in your health due to chronic conditions.

At these times, although your condition may not require admission to a medical facility, receiving medical support at home may provide the help and assurance you and your family members need.

Home health services

Parkview Home Health works in partnership with your physician to provide a wide range of home care services for you or your loved ones, including:

Skilled nursing care to meet your health needs and teach you self-care techniques
Home IV therapies, including antibiotics, total parenteral nutrition, enteral therapy, respiratory medicines and more
Physical, occupational and speech therapies
Medication education and monitoring by nurses and licensed pharmacists
Assistance with your personal care needs by home health aides
A personalized nutrition program developed for you by a registered dietitian
Emotional and social support from medical social worker as you recover from your illness or injury
Ambulatory infusion services

Admissions

A referral is needed for you to begin receiving services. If you are currently at home, your primary care physician or a Parkview Home Health representative can determine whether home health services are appropriate for you. If you are interested in receiving home health care services, talking to your physician is a great place to start.

If you are currently in the hospital or a nursing facility, your physician or a facility staff member can coordinate services for you.

If you are referred for home health services, a nurse will assess your condition and discuss a plan of care.

Meet our team

Parkview Health has providers specially trained in coordinating care for those approaching end of life or who need assistance managing pain or other symptoms. These physicians work with the larger care team to provide services throughout northeast Indiana. Our care team includes physicians, specially trained Advanced Practice Providers (NP/PAs), registered nurses, therapists and others who work together to deliver comprehensive care.
