Refer Your Patient to the Cardiometabolic Clinic

In the Cardiometabolic Clinic at Parkview Heart Institute, patients experience:

• Coordinated care by the clinic care team, beginning with a thorough assessment and followed by additional testing as appropriate, education, treatment and support. At Parkview Heart Institute, all aspects of care will be provided in one convenient location.
• Specialized treatments that deliver cutting-edge technology and an individualized treatment plan that addresses the unique ways that they experience heart disease and diabetes.
• Prescriptions, including state-of-the art medications to improve blood sugar and reduce cardiovascular risk.
• A team approach: physicians, nurses, pharmacists, dietitians, wellness coaches and diabetic educators who will collaborate to help them achieve their goals.
• Education about medications, dietary considerations, and physical activities to best manage their health.

Referrals may be considered when the following are present:

• Younger patients with new or recently diagnosed AFib
• Paroxysmal AFib or atrial flutter with an uncontrolled rate, significant symptomatic episodes or syncopal attacks attributed to AFib Early referral is encouraged before paroxysmal AFib transforms to a persistent state and to avoid complications
• Refractory AFib or intolerant to first-line medication therapy for rate control
• Initiation, monitoring, or titrating of antiarrhythmics, especially membrane active antiarrhythmic agents (e.g., sotalol, amiodarone, dofetilide), the latter of which require careful management
• On-going limiting symptoms despite adequate rate control
• AFib with history of or high risk for embolic stroke (CHA2DS2-VASc score ≥ 2) and with high bleeding risk (HASBLED score ≥ 3)
• Antithrombotic treatment is problematic or contraindicated
• AFib with significant atherosclerotic heart disease including heart failure or myocardial infarction, or with multiple risk factors for heart disease and/or other complex comorbidities
• ECG in sinus rhythm suggesting an electrophysiological disorder (e.g. short PR interval/delta wave)
• AFib with new or underlying structural heart disease
• AFib with suspected valve disease
• Management of medical therapy for AFib in the periprocedural setting
• Other symptomatic tachy or brady arrhythmias

The Cardiometabolic Clinic is a multidisciplinary team of cardiologists, dieticians, and pharmacists, working in collaboration with primary care providers, endocrinologists or other referring providers to assist individuals in maximizing the cardiovascular prevention aspects of their diabetes care.

Our care team will review current diabetic therapies and focus on interventions proven to maximize cardiovascular benefits, such as SGT2 inhibitors and/or GLP-1 receptor agonists. Interventions may also consist of lifestyle modifications such as:

• Making nutritional changes to diet.
• Maintaining control of blood sugar.
• Increasing physical activity.
• Achieving and maintaining a healthy weight.
